Global Senior Care provides a wide range of medical & non-medical home care services in Vancouver to everyone with special needs. We provide four levels of home care which all include physical, cognitive & social activities (PCS activities) for clients who can benefit from these services.
About the Position
For this position, we are looking for someone as a support worker to administer and implement a variety of activities to assists our clients to deal with personal and social problems. The duties and responsibilities of this position include but not limited to the following tasks:
Duties
- Review client background information, interview clients to obtain case history and prepare intake reports
- Assess and investigate client’s eligibility for social benefits
- Work in a team-based environment and provide therapeutic recreation/leisure interventions to designated clients/residents
- Participate as a member of an integrated collaborative health care team, assessing clients’/residents’ recreation and personal care needs
- Assist client with in-home support and program implementation
- Coordinate with other health care professionals to manage health care program recommended by family physician
- Complete progress notes daily and update care plans as required and assessed
- Report to the Director of Care and coordinate client schedules with health care team of caregiver
